Individualized Tuition Forms

Tuition at Sonoma Academy is determined based on a review of your financial situation and on the availability of scholarship funds. Please submit your individualized tuition paperwork at the same time your child submits his/her application for admission. We will then be able to notify you of your tuition level when your child receives his/her admission response. However, you can be assured that admission and tuition applications are reviewed separately.

You will need to review and/or complete the following: (click on form name to download)

  • Sonoma Academy Tuition Policy Statement
  • If parents are divorced/separated, see this form
  • Sonoma Academy Individualized Tuition Application must be sent to Sonoma Academy by January 10, 2008
  • Complete online the School and Student Service (SSS) application form (also known as the PFS, Parents' Financial Statement) by going to https://sss.ets.org/. It is recommended that you complete the paper application in draft form, and then transfer that information to the SSS online form. This will allow us to process your application immediately.
    Notes: Sonoma Academy's SSS school code is 2495. If you own your own business or farm, you will need to complete the Business/Farm Statement.
  • The IRS 4506 which authorizes release of tax forms filed with the IRS.
